Companion Adoption Agreement

Our contract is a lifetime contract for the animal.

If all Information is not completed, we will not process the application. If you are not ready or everyone including spouse or significant other is not 100% on board, please do not apply.

We will check vet info.

If your dogs are not current on vaccines and if all animals are not altered (unless deferred by a vet) then do not waste your time or ours. We will not approve the application.

I agree to allow TRHAP to contact the vet for a reference and any information we deem helpful or needed to make a determination in the adoption process. Please contact your vet to ok the release of information to us.

Are your dogs and cats, including animals living in the home owned by other people (roommates etc), spayed/neutered and current on Dhlppc, Kennel cough and Rabies vaccines and cats spayed/neutered and current on feline vaccines? We will check this information. If dogs are not current on vacciens and if all animails are not altered (unless deferred by a vet), then do not apply. We will not approve the application.

Do you have a fenced yard? Required

If you decide on another animal or if you are not the selected adopter, this application is still valid for another or future adoptions. You do not need to complete another application unless information has changed.


ATTENTION: Please be sure the application is complete. Incomplete applications will not be considered. The completion of this application does not guarantee adoption approval.

4 DX TESTING: Pups under the age of 6 month may not have a 4DX test done. The 4DX tests for Lyme, Ehrlichia, Anaplasma and Heartworm. It takes time for a dog who is bit by a tick or mosquito to actually show up in a positive testing so therefore being done prior to 6 months of age may not be a reliable test result. We recommend that it be done by your vet after 6 months of age.

PUPPY CLAUSE: All puppies (under the age of 6 months) that are unaltered at the time of adoption will be altered by 6 months of age. If this is not done at 6 months of age and there is no medical reason why it could not be done, we have the right to reclaim the pup for failure to abide by our contract. (If the vet recommends altering at a slightly older age or due to medical reasons, we must be notified.) For local adoptions, you MUST use our vet. We will provide a vetting card that you will take to the vet at each visit. Our adoption fees for local adoptions cover initial deworming, Kennel cough vaccine, Rabies vaccine, Dhlppc vaccine and any initial puppy boosters, Lyme vaccine and initial booster, microchip and spay or neuter.

If you need to reschedule the spay or neuter, please call us and we will contact the vet. If you fail to keep appointments and the vaccines need to be restarted, YOU will be responsible for those charges. If you are a NO SHOW for the spay/neuter appointment, you have cost the vets a loss by not being able to schedule someone else in your place.

The adopter is responsible for taking their adopted puppy to the appointment on the date and time it is scheduled and for picking up once surgery is complete.

If you are not local to us, we will require notification when the pup is altered and may require proof of spay/neuter.

RULES AND REQUIREMENTS of The Road Home Animal Project:

  • Dog must be kept indoors as a pet. Not chained outdoors. Not kept outdoors full time in a run/kennel or fence.
  • If adopter can no longer care for their adopted animal they must contact TRHAP immediately and return the animal to us. No one can rehome to a family member or someone they know, nor can they give the dog to a shelter or other agency. Our contract is a lifetime contract for the animal.

The adopter agrees to contact TRHAP for the following reasons:

  1. Prior to being moved to another residence.
  2. If the adopted animal(s) becomes deceased.
  3. If the adopted animal(s) becomes seriously injured or ill.

It is the adopter's responsibility to pay for transport of the adopted animal back to The Road Home Animal Project. All costs upon departing and while in the adopter's care are to be paid by the adopter, including return transportation. TRHAP is not responsible for any accidents or injuries either to the animal, adopter or anyone else after leaving our facility (or the facility of the foster/volunteer housing the animal).

Prohibited Uses: I agree that under no circumstances shall the adopted animal be used for breeding purposes.

If an animal you adopt is pregnant at the time of adoption, the baby/babies automatically attach to this contract and cannot be sold. If the adopter does not wish to keep the baby/babies, they shall be returned to The Road Home Animal Project following weaning. The adopter agrees that a representative of TRHAP may make unannounced visits at any time to any location where the animal is located for confirming that the care required in this contract is being provided. The adopter agrees to hold harmless The Road Home Animal Project, its volunteers, foster care providers, owners of the animal, former owners and directors for any damage or injury caused to any person or property by the animal. The adopter is advised to obtain appropriate liability coverage, as The Road Home Animal Project will not be responsible for liability. If TRHAP becomes aware that the animal adopted is not being adequately cared for or becomes aware of a breach of any of the provisions of this contract, TRHAP maintains the right to immediately and without notice, repossess the animal(s) and maintains the right to enter whatever premises are necessary to affect such repossession. We would like the adopter to send pictures of adopted animal(s) in their loving homes. In the event the adopter is no longer able to care for the animal, the adopter will immediately notify TRHAP. You will only release said animal to TRHAP.

An approved adopter will have a meet and greet. If you cannot take your animal home on the day of your meet and greet for any reason, a $25.00 per day boarding fee will need to be paid starting on that day, if and only if we have an available foster. You will be required to pay the entire adoption fee and boarding fees that day in cash. We DO NOT hold animals who's adoption fees have not been paid. Adoption fees are non-refundable, but are tax deductible.
